What is Corewell Health MyChart?
MyChart gives you secure, online and mobile access to your Corewell Health electronic health record. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can:
- Request medical appointments.
- View your health summary from the MyChart electronic health record.
- View test results.
- Request prescription renewals.
- Access trusted health information resources.
- Communicate electronically and securely with your medical care team.

How do I sign up for MyChart?
You can sign up at your provider’s office, or if you’re 18 or older, you can also create an account on your own. Visit mychart.corewellhealth.org or download the app. Next, create a username and password. After you answer the security questions, your MyChart account will securely connect to your Corewell Health medical record.

Corewell Health patients between the ages of 12-17 can access their own medical records through MyChart, using their own username and password. This can be established by calling MyChart Customer Support or request with a provide.

Can you send me a new activation code if I have lost it, let it expire, or did not receive it?
If you don’t have an activation code, you can request one on the sign-up page. You can also contact us at appsupport@corewellhealth.org. After we verify your information, you will receive a new code.

My activation code does not work, what should I do?
For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, email us at appsupport@corewellhealth.org or you can call MyChart Customer Support at 877.308.5083.

Can I message my doctor in MyChart?
Yes, you can message the office of the Corewell Health providers you’ve seen in the past three years. Your provider should be listed under “New Medical Question”. If their name is not listed, contact the provider’s office and ask to have them added to your "Care Team" as a Primary Care provider or ask to schedule an upcoming appointment or office visit. Messages can be replied to for 30 days.

Where can I update my personal information?
On the Personal Information page in MyChart, you can update your address, phone number, email address, preferred name, and other personal details at any time so that your clinic always has the most up-to-date information in your record.

Who do I contact if my medical information is incorrect or outdated?
To make changes or corrections to your medical information, contact your provider’s office. Your Corewell Health provider maintains the confidential medical information in your account.

When will my test results be available?
You’ll get an email when a new result is in MyChart. Most results are released twice daily, some may take several days, depending on the complexity of the test. In addition, certain sensitive tests are not released to your MyChart account. If you have questions, please call the office of the provider who ordered the test.

Can I link my MyChart account with other health organizations?
You can now use MyChart to view your health information from other organizations where you and your family have records. When you connect your accounts from the participating organizations, you may be able to see the following types of information all right here in your account with MyChart.
To connect your MyChart account with another organization, log into your account and go to the “Menu” then select "Link My Accounts." Select or search for the organization you would like to link to your account.

Can I view a family member's medical record in MyChart?
Yes, you can. MyChart proxy access allows you to access the medical records of your family members and others you care for, with their permission. You must be at least 18 years old to have proxy access to another person’s medical record.
You might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in MyChart.
To view your child's medical information and message their providers, you will need to have them added to your MyChart patient portal via Proxy Access. Parents/legal guardians can request access to children between the ages of 0-17 by submitting the “Request to Access a Minor’s Record” form by logging into MyChart, selecting “Menu,” then choosing “Request Minor Proxy Access” in the Sharing section or by calling MyChart Customer Support.

How is proxy access different for adolescents between the ages of 12-17?
Parents/legal guardians with adolescent proxy access grants access to upcoming appointments, scheduling new appointments, messaging to and from providers, allergies, and immunizations. Corewell Health patients between the ages of 12-17 can access their own medical records through MyChart, using their own username and password. This can be established by calling MyChart Customer Support or request with a provider.
The reason the access to your adolescent’s MyChart account is limited is due to limitations of our electronic medical records system (EMR). Because of Michigan law, when an adolescent turns twelve (12) years of age, certain medical information in their medical record at Corewell Health is protected and the adolescent does not need parental consent to treat for certain services (e.g., services related to pregnancy, sexually transmitted infections, substance abuse, and behavioral health). The relevant Michigan laws are MCL 333.5127, MCL 333.5133, and MCL 722.623.
All other information must be obtained at the provider’s office or through our medical records department.

I forgot my username or password. What should I do?
If you're having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the login fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password. You can also contact MyChart Customer Support at 877.308.5083.

How is MyChart secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged in to our website or mobile app.
You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to MyChart, in addition to using your username and password. For more information, check out our helpful video on two-step verification.
You might receive email or text notifications that contain links to MyChart. These notifications allow you to login to MyChart securely using a link that expires after a short time. Keeping your MyChart account secure means keeping your phone number and email account secure. If you think that your phone number or email account may be compromised, you can change the phone number or email account on file on the "Personal Information" page in MyChart.

MyChart is owned and operated by MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and username will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.
